At Warm Health, we believe that two-way communications lead to better health outcomes. That’s why we developed Warm Health. Warm Health creates the opportunity for you to work closely with your care manager as you take charge of your health!
If you’ve been notified by your health plan that Warm Health is available to you, then you already qualify for one of our health programs. This opportunity comes to you at no cost because it’s a benefit provided by your health plan.
Warm Health offers programs for managing chronic conditions such as diabetes, CHF and asthma. We also believe in preventative care, and offer programs such as; Prenatal, Postpartum and Well Baby health. All of our programs are educational, interactive, fun and good for you!
Throughout your health program, our health expert, MaryBeth, will share tips on how to stay healthy and happy. And, she’ll help keep you in touch with your care manager, who will provide you customer service and support anytime you need it. MaryBeth will ask how you’re doing on a regular basis; your answers will help her learn more about you so she can tailor your health program to meet your needs, give you extra support, or alert your care manager to any health concerns or questions you may have.
Restriction:
You will need to be enrolled in a program by your health plan to use Warm Health.
